Instrumental Lessons Application Form for Instrumental Lessons & Hire 2019-20 (#2) Please complete the following form to apply for instrumental lessons for your child, or child in your care. For additional children or to learn a second instrument/theory, please complete the form again. Signing up for instrumental lessons requires a term’s commitment. You will be charged for a whole term. Fees will be added to your parent pay account within the first couple of weeks of term. Instrumental lessons will automatically continue unless you give notice to stop – you do not need to reapply each term. Notice to stop lessons must be given in writing to the school office or emailed to email@example.com. Following confirmation of availability, successful applications for piano lessons must be immediately followed up with payment of half a term’s deposit via Parent Pay. Instruments must be brought to school on the day of lessons and taken home for practice in between. Your child will practice regularly and sufficiently (starting at 10 minutes a day). Kilmorie School will have the right to terminate the loan if there is evidence of a lack of commitment to learning. The instrument is only to be used by the child named in the application for lessons. Kilmorie School will pay for reasonable wear and tear (repair work). Accessories such as reeds and replacement strings need to be purchased by parents/carers. These are not provided by Kilmorie School. If your child leaves Kilmorie School the loan will be terminated and you will return the instrument to the school. If your child is in Year 6 the instrument should be returned to the school at the end of the academic year. Insurance is not provided by Kilmorie School, and in the event of loss or damage you are responsible for replacing the instrument for one of equivalent value. Insurance can be bought through a number of musical instrument insurers, for example: www.allianzmusicalinsurance.co.uk or www.endsleigh.co.uk.
