Application Form for Instrumental Lessons

Please complete the following form to apply for instrumental lessons for your child, or child in your care. For additional children or to learn a second instrument, please complete the form again.

- Signing up for instrumental lessons requires a term’s commitment. You will be charged for a whole term.
- Fees will be added to your parent pay account within the first couple of weeks of term.
- Instrumental lessons will automatically continue unless you give notice to stop – you do not need to reapply each term.
- Notice to stop lessons must be given in writing to the school office or emailed to - before the end of the current term.
- Following confirmation of availability, successful applications for piano lessons must be immediately followed up with payment of half a term’s deposit via Parent Pay.
